Primary and Community Care Services

Primary & Community Care Services (PCCS) is a health organisation working to make it easier for people to access the health and community services they need. It's about creating a smoother journey for everyone, including the people who deliver health and community services.

We are an approved NDIS Provider and we offer specialist care and coordination services for people who experience:

- Complex health issues

- Social and welfare needs

- Long term mental health issues

- Psychosocial disabilities

 

Some of the services we provide include our Well Together Groups. These are specialised groups providing supported social participation to build well-being, inclusion and resilience. Activities include art or cooking classes, yoga, gardening, photography, films, exercise and more.

Below is some information about two of our groups.

Our Well Together Groups are designed and run by PCCS onsite at The Hub on the Gold Coast, in our Thornleigh office in Sydney or venues in the community. You can also attend these groups via online video.
